The Urgent Need for Primary Healthcare Reform

Nigeria’s primary health care (PHC) system, the crucial first line of defence for our nation’s health, is in crisis. This system, meant to prevent illness and promote wellness for all Nigerians, is failing. It suffers from a crippling lack of funds, crumbling facilities, and a shortage of skilled medical staff.

The consequences are devastating: preventable diseases thrive, and maternal and child mortality rates remain alarmingly high. We need a complete overhaul of our PHC system. We need a transformation that prioritizes the health and well-being of every Nigerian citizen.

The problems are clear and interconnected. Chronic underfunding starves the system, leaving facilities without essential medicines, equipment, and maintenance. Poor infrastructure means many PHC centers are dilapidated, lacking basic necessities like electricity, clean water, and sanitation. The shortage of qualified healthcare professionals, especially doctors, nurses, and midwives, further weakens service delivery. Many leave for better opportunities, leaving rural communities underserved.

These challenges are worsened by poor governance and corruption. Funds intended for PHC are often misused, leading to inefficiency and a lack of accountability. Insufficient community involvement means services often don’t meet local needs.

However, the system can be fixed. Governments at all levels must significantly increase funding for PHC. This means ensuring adequate resources for essential drugs, equipment, and facility upkeep. Strict financial management and mechanisms to prevent corruption are crucial to ensure funds reach their intended purpose.

Existing facilities must be renovated, ensuring they are properly equipped with essential amenities, including reliable power and clean water. Priority should be given to the training and retraining of healthcare workers, especially in rural areas. This includes offering competitive salaries, incentives for rural postings, and opportunities for continuous professional development.

Clear lines of responsibilities should be established, robust monitoring and evaluation systems should be implemented and accountability at all levels should be ensured. Furthermore, local communities should be empowered to participate in the planning, implementation, and monitoring of PHC services.

Government should partner with development organizations, NGOs, and the private sector to leverage resources, expertise, and innovation in support of PHC revitalization.
